Standard of the Quarter
There is a process for evaluating the effectiveness of the patient blood management (PBM) program. This evaluation is integrated into appropriate quality review activities within each institution. The information is used to improve anemia management, minimize blood loss and improve transfusion practice. This evaluation is based on metrics defined by the hospital.
SABM continually emphasizes the benefits of ongoing research and it follows that one of our important goals is to review or analyze the positive impact of innovative PBM programs. Our close association with leading international health organizations means that we have our finger on the pulse of the most current needs and technologies. The reader’s attention is directed to our PBM EZINE (November 2016), the abstract “COST EFFECTIVENESS” which reports on provable cost savings that are ever more important to the success of PBM programs as hospital administrator’s grapple with the changing financial landscape. The abstract “PATIENT BLOOD MGMT/PROGRAMS” appropriately draws attention to the trauma patient and improved outcomes by implementation of PBM. These are just two examples of quality research readily available on the SABM website.
As part of research or evaluation, indicators 4.3 and 4.4 emphasize data collection. This may include such metrics as the number of units of red cells, platelets, plasma, and cryoprecipitate that are transfused, percentage of inpatients transfused, also the average hemoglobin or hematocrit before and after transfusion. Other data might include the number of patients enrolled in preoperative anemia management, the use of erythropoietic stimulating agents or intravenous iron, pharmacologic agents, and evaluation of procedural blood loss including phlebotomy. Innovative PBM programs have seen much benefit from the evaluation of overall transfusion practices by clinical service or section (e.g., Surgery, Orthopedics, etc.), and by individual ordering physician. This can provide physicians with an opportunity to see how their transfusion decisions compare to their peers, encouraging improvement in transfusion decisions.
Comprehensive PBM consists of multiple patient-centered strategies that improve clinical outcomes. Accordingly, program evaluation and quality improvement involves much more than transfusion utilization review and of necessity would include systematic evaluation of the various treatment modalities available. The start of a new year provides an excellent opportunity to review our individual PBM programs, benefitting from the resources SABM makes available to members with measurable improvements in the patient care provided at our hospitals.
